BHB Unpacked: A UK Dietitian on What Exogenous Ketones Really Do Inside You

If you've spent any time browsing keto supplements, you've seen the three letters: BHB. Beta-hydroxybutyrate crops up on tubs, gummies and powders across every UK health shop, usually accompanied by bold claims about instant ketosis and boundless energy. But how much of it is genuine physiology, and how much is clever marketing?

To find out, we spoke with Dr Sarah Whitmore, a London-based registered dietitian who researches ketogenic nutrition, and asked her to break down the science in plain English.

First Things First: What Actually Is BHB?

'BHB is what's known as a ketone body — one of three your liver produces when carbohydrate intake is very low,' Dr Whitmore explains. 'When your body runs out of glucose to burn, it switches to fat, and the liver converts some of that fat into ketones. BHB is the most abundant of these, and it's what your brain and muscles can use as fuel.'

So far, so natural. The twist with supplements is that most products contain exogenous BHB — meaning it's made in a lab, typically bound to minerals like sodium, potassium or calcium to form 'ketone salts.'

What Happens When You Take a BHB Supplement?

'Within about 30 to 60 minutes, blood ketone levels do rise,' says Dr Whitmore. 'That part is measurable and real. The question is what that rise actually achieves — and this is where I urge people to be careful.'

She points to a few areas where the evidence is genuinely interesting:

Where BHB Shows Real Promise

Easing keto flu. 'When people first cut carbs, they often feel foggy, headachey and drained — partly because they're losing sodium and water. Exogenous ketones may provide an alternative fuel while the body adapts, and the mineral salts themselves can help replace electrolytes. There's a reasonable physiological rationale here.'

A mental and physical boost mid-transition. Some small studies suggest BHB can support cognitive performance and exercise output during the early days of a ketogenic diet, before full fat-adaptation kicks in.

Appetite signals. 'There's emerging research suggesting ketones may influence hunger hormones like ghrelin. It's early days, but it's a genuinely interesting area.'

Where the Hype Outruns the Science

'Here's the crucial bit,' Dr Whitmore stresses. 'Taking BHB does not put you into nutritional ketosis in the way the marketing implies. Yes, your blood ketones rise — but if you've just eaten a sandwich, your body still has plenty of glucose to burn. The ketones don't override that.'

In other words: BHB is not a cheat code. 'You cannot take a ketone powder and carry on eating 200 grams of carbs a day and expect fat loss. The underlying diet still does the heavy lifting.'

She's equally sceptical of claims that exogenous ketones directly 'melt fat.' 'Ketones consumed as supplements are themselves a fuel source — roughly a calorie per gram. Your body may burn them before it burns your own stored fat.'

Who Might Genuinely Benefit?

Dr Whitmore's honest take: 'For someone already following a well-formulated keto diet, BHB is a nice-to-have, not a need-to-have. Where I see a sensible use case is during the first week or two of transition, for people who struggle with keto flu, or athletes doing fasted training who want a fuel source without carbs. And for anyone on a strict keto diet missing electrolytes, the mineral salts are a genuine bonus.'

She also flags a practical caveat: 'The sodium content can be significant. Anyone with high blood pressure or kidney concerns should speak to their GP first, and everyone should check labels — some UK products vary wildly in dose and quality.'

The KetoBase UK Verdict

BHB is real chemistry with real — if narrow — applications. It won't replace diet discipline, but as a bridge through the keto flu and a convenient electrolyte source, it earns a measured thumbs-up from the science. As ever: food first, supplements second, and always read the label.
